چکیده

Critically ill general medical patients are an increasing group in the Emergency Department (ED). This register-based cohort study aimed to examine these patients’ characteristics, ED management and outcome, investigate factors associated with ICU admission. The comprised all adult triage 1 treated by a specialized multidisciplinary team 2015 2016. Univariate multivariate analysis were used. 1294 included. Mean age was 59 years, 56% (n = 725) male, mean National Early Warning Score 2 (NEWS2) 7, intensive care unit (ICU) admission 56.8% 735) mortality rate 16.8% 217). Median length of stay (LOS) 1.6 h, 1.2 h if admitted ICU. most frequent discharge diagnosis acute poisoning (24.0%, n 308). Younger age, male gender, arriving at nighttime weekdays, higher NEWS2 arrival, critical interventions or medications More than half ICU, 16.8%. A large proportion diagnosed poisoning. NEWS short LOS suggests that is beneficial.

Emergency team calls for critically ill non-trauma patients in the emergency department: an observational study

BACKGROUND Handling critically ill patients is a complex task for Emergency Department (ED) personnel. Initial treatment is of major importance and requires adequately experienced ED doctors to initiate and decide for the right medical or surgical treatment. Our aim was, with regard to clinical presentation, management and mortality to describe adult non-trauma patients that upon ED arrival eli...

The ABCDE primary assessment in the emergency department in medically ill patients: an observational pilot study.

BACKGROUND Competency in the Airway Breathing Circulation Disability Exposure (ABCDE) approach is required for working in the emergency department. There is limited knowledge on how often and how completely the ABCDE approach is applied to medical patients. The objectives of this study were to assess the frequency with which the ABCDE approach was used in potentially unstable patients and to de...

Bench-to-bedside review: Outcome predictions for critically ill patients in the emergency department

The escalating number of emergency department (ED) visits, length of stay, and hospital overcrowding have been associated with an increasing number of critically ill patients cared for in the ED. Existing physiologic scoring systems have traditionally been used for outcome prediction, clinical research, quality of care analysis, and benchmarking in the intensive care unit (ICU) environment. How...

Outcome of postoperative critically ill patients with heparin-induced thrombocytopenia: an observational retrospective case-control study

INTRODUCTION Heparin-induced thrombocytopenia (HIT) is described as a decrease in platelet count associated with heparin administration and is an immune-mediated adverse drug reaction that can cause both arterial and venous thromboses. It can be a life-threatening complication of heparin exposure. Little data concerning incidence, predisposing factors, or outcome in critically ill surgical pati...

Light and the outcome of the critically ill: an observational cohort study

INTRODUCTION Light before and during acute illness has been associated with both benefit and harm in animal models and small human studies. Our objective was to determine the associations of light duration (photoperiod) and intensity (insolation) before and during critical illness with hospital mortality in ICU patients. Based on the 'winter immunoenhancement' theory, we tested the hypothesis t...
